Understanding Extracorporeal Therapies for Pets: More Than Dialysis
Extracorporeal therapies process a pet's blood outside the body to treat kidney disease, toxins, immune disorders, and some cancers.
When a pet is critically ill, medication and standard hospitalization are not always enough. Extracorporeal therapies process a pet’s blood outside the body to remove harmful substances, support organ function, or help manage certain complex diseases.
Dialysis is the best-known example, especially for pets with kidney failure. But extracorporeal therapy also includes treatments for certain toxin exposures, immune-mediated diseases, and newer cancer-related protocols.
At select MedVet locations, our veterinary teams use these therapies when a pet’s condition calls for support beyond traditional treatment.
What Does Extracorporeal Therapy Mean?
“Extracorporeal” simply means outside the body.
During treatment, blood is carefully circulated through specialized equipment, filtered or processed to address the specific problem, and then returned to the pet.
What happens during that process depends on what is being treated. Some therapies act as temporary support for an organ. Others remove toxins, antibodies, or other substances that are causing harm.

Is Dialysis the Same as Extracorporeal Therapy?
Dialysis is a type of extracorporeal therapy, but the terms are not interchangeable.
Dialysis helps when the kidneys cannot adequately filter waste products or maintain normal fluid and electrolyte balance. When the kidneys suddenly stop working properly, a pet can become very sick. In some cases, dialysis can temporarily take over the kidneys’ filtering role while veterinarians treat the underlying cause and watch for recovery.
Unlike in human medicine, where dialysis may be long term, veterinary dialysis is often used as temporary support during a critical illness or acute kidney injury. Kidney support is just one application. At MedVet, extracorporeal therapies are also for conditions involving toxins, immune-mediated disease, and cancer treatment.
How Extracorporeal Therapies Help Remove Toxins
One of the most time-sensitive uses of veterinary extracorporeal therapy is removing certain toxins from the bloodstream.
Pets can accidentally ingest medications, household products, or other substances that can be life-threatening. In some cases, treatments such as inducing vomiting or giving activated charcoal may not be enough.
For certain toxin exposures, a therapy called hemoperfusion may be considered. During hemoperfusion, blood passes through a specialized filter that binds the toxin. The treated blood is then returned to the pet.
Hemoperfusion is often used to treat medication toxicities, including non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or naproxen, muscle relaxants such as baclofen, and nutritional supplements like vitamin D.
Timing is critical. These therapies only work while the harmful substance is still in the bloodstream, and that window can be shorter than most people expect. For a common drug like ibuprofen, the optimal window for hemoperfusion may be within the first two hours after ingestion. Once a toxin moves into the body’s tissues, it becomes much harder to remove. Do not wait for symptoms to appear before calling for help.
If your pet has a known or possible toxin exposure or is showing signs of sudden serious illness such as collapse, severe vomiting, or trouble breathing, seek urgent veterinary care immediately. Early evaluation gives your veterinary team the best chance to choose the right treatment.

How Therapeutic Plasma Exchange Helps Pets With Immune-Mediated Disease
Some diseases occur when the immune system mistakenly attacks a pet’s own body. These are called immune-mediated diseases.
Examples include:
- Immune-mediated hemolytic anemia (IMHA)
- Immune thrombocytopenia (ITP)
- Myasthenia gravis
- Certain inflammatory or autoimmune conditions
In these diseases, harmful antibodies or immune-related proteins circulate in the plasma, which is the liquid portion of blood.
Therapeutic plasma exchange, or TPE, helps remove these substances from circulation. During TPE, blood is separated into its parts. The plasma containing harmful antibodies or immune proteins is separated from the red blood cells and replaced with replacement fluids and/or donor plasma, depending on the patient’s needs. The red blood cells are returned unaltered to the pet.
The goal is to lower the amount of harmful immune activity in the bloodstream while other treatments, such as immune-regulating medications, begin working.
TPE is typically considered in patients with severe disease or those not responding as expected to standard treatment.

New Possibilities: Extracorporeal Therapy and Cancer Treatment
One of the more significant newer uses of extracorporeal medicine involves treatment for dogs diagnosed with osteosarcoma, a type of bone cancer. With conventional treatment for osteosarcoma (amputation), the average survival time is typically three to six months. At MedVet Columbus, specialists perform a specialized apheresis procedure as part of an advanced treatment protocol available at only a limited number of veterinary centers nationwide.
Through a process called mononuclear cell collection (MNC), veterinary specialists collect specific white blood cells from a patient’s blood. Those cells are then processed to target specific cancer cells. In some patients who qualify for and complete the full protocol, survival has extended to two years or more.
Results vary, and this is still an evolving area of veterinary medicine. For some qualifying patients, this approach may offer a far longer lifespan than conventional treatment alone. If your dog has been diagnosed with osteosarcoma, ask your veterinarian (before amputation) whether this treatment, called ELIAS Cancer Immunotherapy, may be an option.
What Happens During an Extracorporeal Therapy Treatment?
Every pet’s treatment plan is based on their diagnosis, overall health, and specific needs. These therapies are chosen because they fit a specific disease process or treatment goal, not simply because a pet is critically ill.
Before recommending an extracorporeal therapy, a veterinary specialist considers the underlying disease or toxin exposure, the pet’s current condition, whether the therapy is likely to help, and other available treatment options.
During treatment, pets are closely monitored by veterinary professionals. Because extracorporeal therapies require specialized equipment and training, they are performed at veterinary specialty hospitals with advanced critical care capabilities.
